An essential part of your immigration paperwork, your immigration medical evaluation provides important information about your medical history and current health.
In order to be admissible to the United States, you need to show that you have no communicable disease of public health significance, no missing vaccinations, no physical or mental disorders associated with harmful behavior, and no current issues with drug abuse or addiction.

Reviewing your documents
Before your immigration medical evaluation appointment, you need to complete a Document Review Form. At MIMPC, we offer document review services for a $49 fee per person.
In your document review session, we give you a personalized timeline for your immigration physical process. This personalized approach helps you understand your case better when you meet one-on-one with our Civil Surgeon.
Your review session tells you more about the vaccinations you still need and the cost of those vaccinations. We also start your mandatory labs at the review appointment, getting you a head start in the immigration medical evaluation process.
Gathering your records — what you need to bring
Please bring a valid passport or other government-issued photo identification to your immigration medical evaluation appointment. Also bring your I-693 immigration paperwork.
You should bring your health records, including vaccination records, for your Civil Surgeon to review. You need to be up-to-date on vaccinations in order to immigrate. If you don’t have documented vaccinations, you must receive additional vaccinations.
You also may need to provide documentation on your health history. You should report any special requirements and provide a list of any current medications you take. Bring your tuberculosis certificate from your doctor if previous skin tests showed you being positive for tuberculosis. Bring a signed certificate of clearance if you’ve been treated for syphilis.
Document any history of violent or harmful behavior, and bring treatment records for treatment related to alcohol or drug abuse as well as mental or psychiatric illness.
You can present your health and vaccination records at your appointment. However, we prefer for you to take the time to securely email us a copy of your records before your appointment so we can get a head start on your review.
Personal needs for your exam
When you come in for your immigration medical evaluation appointment, choose clothing that’s loose-fitting and comfortable. Make sure you’re well hydrated and prepared to give a urine sample if you’re consenting to a laboratory evaluation. Please note that laboratory evaluation incurs a separate, additional fee.
We speak English and Spanish. If you don’t, we ask that you please bring along an adult interpreter. Your interpreter needs to come to all of your office visits relating to your immigration medical evaluation. Your interpreter also signs your I-693 immigration paperwork, attesting that they are fluent in your language.
